I, the undersigned, hereby acknowledge that I have voluntarily applied to participate as a member of Iron Cross Fitness.
I understand that use of the gym facilities and equipment involves inherent risks, including but not limited to physical injury, illness, or death. I voluntarily assume all such risks and accept full responsibility for any injury or damage that may occur.
I release, waive, discharge, and covenant not to sue Iron Cross Fitness, KWI, their officers, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained while using the gym facilities.
I certify that I am physically fit and have no medical condition that would prevent me from safely using the gym equipment. I agree to follow all posted rules and instructions regarding gym use.
